Technology User Agreement
- Must by signed every year by
all staff and students before using any networked computers.
- Must include three signatures (teacher,
parent, student).
- Teachers need to collect from every
student in your class and return to Lab with a class list
checked off.
- Will be filed in computer lab under
classroom teacher’s name.
- As soon as your completed forms are turned
in, students will be able to use computers at Mt. View.

* It must be renewed annually by all staff and students.
