Please note that all four forms are required for school registration. Forms can be returned directly to the schools.

Out of Boundary: If you live outside the boundary of the school you wish to register at, you will need to apply for a variance.

Forms are available in other languages online at the Washington State Department of Health web site, which can be found at http://www.doh.wa.gov/cfh/Immunize/schools.htm.

In addition, school/age specific information may be necessary to complete the registration process. Please contact the specific school for details/requirements.
